Puzzle-piece collection and assembly belong to Final Mix. Their absence in original KHII is an edition difference, rather than an incomplete journal category.

Recipes and Creations

The original guide’s first synthesis-list milestone requires Moogle level 9. Its synthesized-recipes collection item concerns the Creations menu, not simply finding recipe documents.

Roxas’s minigame performances do not fill Sora’s journal. In original KHII, playing an activity and reaching its mission target are separate checks.

A collected journal and an unopened entry

For HD 2.5 ReMIX, Square Enix says to open every collected journal entry if the journal trophy has not unlocked. Check this after collection; it cannot replace missing requirements.
